Athletes must be registered through NorthShore Google Form. You will receive a copy of your Google Form after you have registered.
Step 2: Click here for Medical Release
Athletes must have a hard copy of the completed medical form at check-in on the day of try-outs.
Step 3: Click here for Membership Card or
Click here for Tryout Card
Athletes who played for NorthShore for the 25-26 season must bring a copy of their current JO membership card.
Athletes who did not play for NorthShore during the 25-26 season must bring a TRYOUT Card.
Step 4: Bring a $20 try-out fee.
If the athlete has a neon NorthShore shirt from a previous try-out, please wear it.
