- $20 Tryout Fee - Cash or Check (made out to NorthShore Volleyball Club)
- MUST BRING a Completed copy of USAV Medical Release.
- Scroll down to the bottom section and find Medical Release (Click on the blue print)
- MUST BRING a Junior Tryout Membership Card-
